We believe that proper medical care stems from a thorough and respectful understanding of patients as individuals. Because of this, we do not participate in any managed care plans, which allows us to spend the appropriate amount of time cultivating strong, personalized relationships with our patients. We always provide our patients with their full receipts, so they may submit them to their insurance companies for reimbursement if their plan allows for it. Our staff is available to assist you with this process.

For many insurance plans we would be considered an out-of-network provider. PPO and Indemnity plans may cover our services, but you will likely be subject to copays and deductibles.


Our office has chosen to opt-out of Medicare, so our Medicare patients are charged our normal fees. Though our bills cannot be submitted to Medicare for reimbursement, you may submit them to any other primary or secondary insurance that you may have.

Please note that Medicare patients must sign the below agreement before receiving any medical care from us. The agreement states that you are aware that we have opted out of Medicare and that your bills should not be submitted to Medicare.

If you are insured through Medicare, please print out and bring a signed copy of your Medicare Opt-Out contract to your visit.

Surgical Fees

At the time of your pre-operative consultation, we require full payment of the anticipated surgical fee. We will assist you by filing a claim with your insurance company after the procedure. You can expect your insurance company to reimburse you directly according to their particular schedule. For more information, kindly read here:

It is part of our policy that we receive payments in full at the time of your office visit. If you require accommodations, please make arrangements with the office manager prior to your visit.

We will provide you with a full receipt when your payment is made. This receipt can be submitted to your insurance company for reimbursement. Our office manager is always available to answer any questions and is happy to assist you in this process.
